Now Hiring Full:Time Hospice/Palliative/Primary Care Nurse Practitioner/NP Access Hospice : 4933 S 1500 W Suite 200 : Ogden, Utah 84405 Signature Team Video : Click Here The Hospice/Palliative/Primary Care Nurse Practitioner, with a multi:disciplinary clinical team, provides initial and ongoing clinical assessment for patients with Hospice orders, advanced and or progressive medical conditions . Assists in the evaluation and treatment of patients in the Hospices or Palliative Care Program. Conducts patient interviews, and collects and interprets diagnostic data. May perform physical examinations and specialized procedures under the direction of a physician. Documents the delivery of care in the individuals medical records. Provides patient education, and trains other health care professionals. Performs related responsibilities as required.Performs bedside hospice or palliative care patient/family evaluationAssists with diagnosis, treatment and management of acute and chronic health conditionsOrders, performs and interprets laboratory and radiology tests within scope of professional practicePrescribes medications including controlled substances to the extent delegated and licensedOrders treatments and durable medical equipment as indicatedPerforms other therapeutic or corrective measures as indicated, including urgent careConsults with palliative care physician or designees as needed, informs primary physician of services provided and collaborates with other physicians as neededAssists in all facets of care coordination for palliative care referralsPrepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement for services providedComplies with applicable laws and regulations with respect to collaborative agreementsInitiates reimbursement for services renderedProvides training and continuing education for staffAssists in development of clinical practice guidelines/standards in support of quality palliative careAdheres to the practice of confidentiality regarding patients, families, staff and the OrganizationActive State LicenseNPI Desire to be in a patient:centered, healthcare settingExcellent bedside manner with solid customer:oriented presentation skillsCapable of multi:tasking in a fast:paced environment, where you are often moving from station to stationAbility to working autonomously and adapt to changing environmentsTalent for cultivating a positive and energetic team atmosphereStrong computer proficiencyFlexibility to travel by car to community events within 1:4* hours from home locationMust have a valid Drivers License with a clear driving record ANCC or AANP board certified requiredAbility to lift up to 50 lbs. on a consistent, daily basisActively or recently participated in service to Medicare patients and Medicaid patients (preferred)Signature Companies is an Equal Opportunity Employer Please apply on line for this position here: Lee WhitlockSignature Services : Director of RecruitingTel: C: F: Home Health Home Care
